2007 Educational Incentive Program

Purpose

The purpose of the Clinical Coordinator Educational Incentive Program is to recognize the extra effort required by the Clinical Coordination staffs at each of the AZEPTIC Study sites. Knowing that the execution of the screening and enrollment plan along with the trial protocol requires dedication, commitment and time, Ash Access Technology, Inc. is pleased to contribute financial resources to an Educational Fund at each site based on each site's level of achievement. The resources received by each site are intended to be used at the site's discretion, to offset costs incurred for the education and professional development of the site's Clinical Coordination staff

Program Details

Participation in the Educational Incentive Program is not mandatory. Sites should comply with all Site SOP's and obtain approval from necessary personnel as a condition of participation. An acceptance form will be sent to each site and must be signed and returned to Averion for participation in the program.

All incentive monies earned in the first enrollment incentive program released on September 25, 2006 for enrollment activity prior to December 31, 2006 will be honored in addition to those earned in this program.

A payment of $1500 will be made to the Educational Fund of each site whose enrollment meets the target as established by the Sponsor (Appendix A) on or before June 15, 2007. (“Enrollment” is defined as the number of patients who have been randomized and received at least one treatment of Study device. The date of the first treatment on the Study will be considered the date of enrollment. Only data recorded on the Enrollment Log and submitted to Averion will be considered.

Should a site exceed its enrollment target, an additional payment of $75 for each patient enrolled in excess of the target on or before June 15, 2007, will be made to the Educational Fund of that site.
